Move-Up Consent Form
Any In-House player that is moving up to a travel team older than their current age is required to submit a
Play-Up Consent Form.
Please fill out and return to the Registrar.
Reminder
Smoking is not permitted on or near the soccer fields. Smoking will only be permitted in the parking area.
Also, our soccer fields are part of the South Allegheny property. Dogs are not permitted on school property.
